A clear ortho retainer has no pontic it is a standard vacuum-formed clear appliance used purely for tooth position maintenance in patients with a complete arch. An Essix retainer with pontic includes a built-in pontic for patients who are also missing a tooth and need esthetic interim replacement alongside retention. If your patient has a complete arch, order the clear ortho retainer. If they are missing a tooth, order the Essix retainer with pontic.
No. A clear ortho retainer is fabricated from thin thermoplastic material designed for retention — not bruxism protection. Grinding patients will wear through the material quickly and may distort the retainer's fit. For patients with bruxism, prescribe a separate night guard alongside the retainer. Current evidence supports indefinite nightly retainer wear for life after the active retention phase. The standard protocol is full-time wear for the first 6–12 months post-treatment transitioning to nightly wear long-term. Teeth shift throughout life in the absence of retention there is no defined point at which a patient can stop wearing their retainer and reliably expect stability. Set this expectation clearly at debond.
